Succeeding improvements on items that are 90+ quality hasn't completed any of these goals, despite the requirements saying to "improve a single piece of [item] to at least [quality]".

This has been the case in the old goal system as well. It doesn't seem like a bug, just the way the goal works: it checks when you pass the threshold from 89.99 to 90ql or from  98.99 to 99ql or whichever it is.
